If you are sending a written complaint, you might like to use the form provided on this page. This will help you organise your material in a logical way. It is also important that you give us all your information at the outset.

For more detailed information about the complaints process generally, see Complaints to the CMC.

What should you include in your complaint?
Tell us about anything you think may involve misconduct affecting the public sector. You do not need proof, as long as you have reason to believe that misconduct has occurred. If in any doubt, ask us for advice (see contact information above).

Where possible, please tell us:

  • the name, job title and workplace address of the person you suspect of misconduct
  • details of relevant events, where they happened, and when they happened (date and time)
  • who was involved
  • the names of witnesses or other people who may be able to back up what you say
  • any other evidence that supports your view such as photographs, video footage or audio tape-recording, departmental correspondence, applications, contracts, accounts or diary notes
  • any steps you have taken already to resolve the complaint
  • if you have reported the matter to any other agency
  • what action or outcome you would like to see as a result of your complaint.

Not all of the above will be relevant to your particular complaint. However, you need to include sufficient information to allow us to assess your complaint.

Can you make an anonymous complaint?
You don’t have to give your name, although anonymous complaints are more difficult to deal with. At the very least, leave some name by which we can reach you and your contact phone number in case we need to ask you any follow-up questions or so that we can let you know the outcome of your complaint. Alternatively, provide a postal address.

If you make an appointment to visit the CMC
Organise your material in a logical and concise way. Refer to the prompts above to decide what is important.

Bring any relevant documents or other information such as the names of people who are involved or are witnesses; relevant dates and reference numbers; and other evidence such as photographs, video footage, departmental correspondence, applications, accounts or diary notes. Be prepared to explain how the documents and other information relate to your concern.

Please be honest and reasonable when dealing with us, and treat CMC staff with courtesy. We need to work together if we want to reduce misconduct in our society.

Will your information be treated confidentially?
We would like to give the agency concerned the information you provided, with your name if possible. If you are concerned about the consequences of giving your name, please let us know.

We will do our utmost to treat your information confidentially. However, our ability to do so will depend on the seriousness of the subject matter, the question of public interest, FOI legislation and other legal processes.

In some cases you may be required to answer further questions or give evidence.
